We kindly request that all regular attenders and Covenant Partners individually complete this form as we update our contact records and prepare a directory. You'll be asked to name your spouse if that is applicable, but spouses should complete their own form.

Please complete this form no later than June 15th, even if you do not wish to be in the directory.

If you would like to be included in the directory, please sign up for a time slot to take your photo using this link. This photo would include those in your household who are part of First and Calvary. These photos will be taken by our staff and portraits will not be available for purchase. But the files will be shared for your use.
